Cardiovascular disease is the leading cause of death in America for both men and women, but it is preventable and treatable. The best prevention is awareness. The Christ Hospital offers free online surveys to help you determine if you might be at risk for heart disease or vascular disease.

Each survey takes about seven minutes to complete. Having your recent cholesterol numbers and blood pressure readings is helpful, but not necessary, to complete the survey. Your personal and family health history will also be taken into account.

Upon completion of the surveys, be sure to click for your report that will provide your percent risk. If you are deemed at risk, you may be offered a FREE follow-up appointment with a cardiac clinician to review your risk factors and heart health habits.
