The terms of this Notice of Privacy Practices apply to The Christ Hospital, The Carl and Edyth Lindner Center for Research and Education at The Christ Hospital, The Christ Hospital Medical Associates, The Christ Hospital Cardiovascular Associates, and The Christ Hospital Medical Specialists, each of which are legally separate covered entities and are under common ownership and control. For the purpose of this Notice, these five entities have elected to become a single Affiliated Covered Entity that will be referred to as The Christ Hospital. The members of this health care system will share protected health information of patients as necessary to carry out treatment, payment, and health care operations as permitted by law.
II. We have a legal duty to safeguard your protected health information.
We are required by law to maintain the privacy of our patients’ personal health information. We call this information “protected health information” or PHI for short. We must provide patients with notice of our legal duties and privacy practices with respect to PHI. We are required to abide by the terms of this Notice so long as it remains in effect. We reserve the right to change the terms of this Notice of Privacy Practices as necessary and to make the new Notice effective for all PHI maintained by us. You may receive a copy of any revised notices at The Christ Hospital, or a copy of the current Notice of Privacy Practices may be obtained by mailing a request to The Christ Hospital HIPAA Privacy Officer, Attention Medical Record Services, 2139 Auburn Ave., Cincinnati, OH 45219. If you believe your privacy rights have been violated, you may file a complaint with The Christ Hospital HIPAA Privacy Officer, Attention Medical Record Services, 2139 Auburn Ave., Cincinnati, OH 45219. The complaint must be filed in writing. You may also file a complaint with the Secretary of the U.S. Department of Health and Human Services in writing within 180 days of an alleged violation of your rights. There will be no retaliation for filing a complaint.

This Notice of Privacy Practices is effective April 14, 2003, with revisions effective April 12, 2010.
